It is dangerous to guess what kind of boiler controls they are using. However, the fatality was an I&E technician and he was part of team attempting to restart the boiler. Smells like older technology controls without diagnostics and reporting capabilities. They were likely operating from a local control panel instead of a remote control room, which would explain why everyone was in close proximity when the incident occurred.
